Ameriflex administers the Benefit Convenience Card (BCC) used with certain Carewell SEIU 503 Healthcare Cost Assistance benefits.
Use this page if you need help:
✓ Activating or managing your card
✓ Checking your available balance or payment history
✓ Understanding why your card was declined
✓ Replacing an expired, lost, or stolen card
✓ Submitting a reimbursement claim
✓ Determining whether to contact Ameriflex, Carewell Benefits, or your insurance carrier
The BCC may only be used for expenses covered by your specific Healthcare Cost Assistance benefit. Coverage rules differ for Marketplace, Medicare, and military-connected benefits.
Keep your receipts, Explanation of Benefits documents, and other proof of payment in case Ameriflex requests documentation.
Before attempting a payment:
✓ Confirm that your Healthcare Cost Assistance benefit is active.
✓ Check the expiration date printed on your card.
✓ Confirm that the expense is covered by your specific benefit.
✓ Check your available balance through MyAmeriflex.
✓ Make sure Carewell has your current health-insurance and contact information.
Your Benefit Convenience Card is for your own eligible expenses. It cannot be used to pay another person’s medical expenses.
For Marketplace plans, the card may be used for eligible premium payments and covered out-of-pocket expenses. The rules are different for Medicare and military-connected benefits, so review the guidance for your type of coverage before making a payment.
First, check your available balance and recent transactions through your MyAmeriflex account or the MyAmeriflex mobile app.
If enough money is available, ask your insurance carrier to try the payment again.
If there is not enough money available to pay the premium, contact Carewell Benefits immediately at 1-844-503-7348.
Continue paying required premiums by the deadline using another payment method when necessary. A declined card does not automatically extend your insurance company’s payment deadline.

Tell the representative that you are an HCW, PSW, or PCA receiving Carewell SEIU 503 benefits. Have the following ready:
✓ Your Benefit Convenience Card
✓ The date of the attempted payment
✓ The type of service or expense
✓ A description of what happened
✓ Any receipt, bill, or Explanation of Benefits related to the expense
If your card is lost or stolen, report it to Ameriflex as soon as possible. You can request a replacement through your MyAmeriflex account or by calling 1-888-868-3539.
Your card expires when:
You are no longer eligible for Healthcare Cost Assistance; or
The expiration date printed on the card is reached,
whichever happens first.
If you remain eligible, Ameriflex generally sends a replacement approximately one month before the expiration date printed on your current card.
Make sure Carewell and Ameriflex have your current mailing address. If your benefits were temporarily turned off, do not discard an unexpired card—it may work again after you regain eligibility and complete any required Carewell steps.
If you paid an eligible expense using your own money, you may be able to submit a reimbursement claim through your MyAmeriflex account.
Keep and submit the documentation Ameriflex requests, which may include:
✓ An itemized receipt or bill
✓ Proof that you paid the expense
✓ An Explanation of Benefits (EOB)
✓ The date and type of service
✓ Information showing that the expense was for you
Expenses are generally connected to the calendar year in which the covered service was received. Do not use the current year’s Benefit Convenience Card balance to pay a bill for a service received during a previous year. Use Ameriflex’s reimbursement process instead.
Ameriflex determines whether an expense and its documentation meet the reimbursement requirements.
Use your MyAmeriflex account or the MyAmeriflex mobile app to:
✓ Check your available balance
✓ Review transactions and payment history
✓ View the status of your card
✓ Submit and track reimbursement claims
✓ Upload requested documentation
✓ Report a card lost or stolen
✓ Request a replacement card

Your MyAmeriflex website and mobile-app login credentials are the same. Contact Ameriflex if you receive an account-registration error or cannot access your card information.

✓ Lost, stolen, expired, or replacement cards
✓ Reimbursement claims submitted to Ameriflex
